Obtain Physician Documentation

Minnesota’s tint exemption is physician-driven, not form-driven. You need a written statement from a licensed Minnesota physician (MD or DO) certifying your medical condition requires darker window tint. There is no standardized DVS form to download — the physician creates the documentation.

Visit Your Doctor

Schedule an appointment with a licensed physician at Mayo Clinic, University of Minnesota Health, M Health Fairview, or your primary care provider. The doctor must confirm your condition and write a medical necessity statement specifying that you require window tint below the 50% VLT limit. Most PCPs won’t sign tint-specific paperwork, which is why many patients use our online service instead.

Ensure Proper Documentation

Your physician’s letter should include: their name, medical license number, practice address, your diagnosis, a statement of medical necessity for darker tint, and their signature. This document serves as your legal exemption under MN Stat §169.71 subd. 4.
